Bring Your Completed Enrollment Packet

Bring your completed Enrollment Packet to the Center for Community & Civic Engagement to be enrolled in the Independent Module course.  

Make sure to have all portions filled out:

-- Student Information Section & Agreement with signature

-- Faculty Advisor Agreement with signature

-- Service Location Information with signature 

-- Assumption of Risk & Release of Liability

You cannot be enrolled anywhere else on campus for this course, nor should you attempt to self-enroll.  It is imperative that our department confirm that you will be serving at a pre-approved service site.
