Experiential Education Assumption of Risk & Release of Liability - This form must be completed and turned in to the class SLA or the Center for Community & Civic Engagement before the student starts their Service-Learning hours.

Enrollment Form - This form confirms that the student has been approved to serve at the site, allows the student and supervisor to outline responsibilities, and includes conduct policies for both the student and site supervisor to sign off on. This form must be completed and turned in before the student starts their Service-Learning hours.

Service-Learning Certificate

Students who complete at least the minimum service-learning requirement for their course are eligible to receive a certificate. To receive a certificate, students must complete all required paperwork.
